It is a secondary school game where friends would constantly make that gesture with their hands. Anyone who views it gets hit in the testicles.
You would put it in inconspicuous places like on your shoulder or fixing the leg of your pants. It is basically a game that is sadistic and hurts a lot if you get caught.
